Request a Transcript

• Hawai'i State Community School for Adults - McKinley Campus, or
• Hawai'i State Community School for Adults - Kaimuki Site (formerly Kaimuki Community School for Adults)

To request an official transcript, please complete the Transcript Request Form below and follow all instructions.

• Complete the Transcript Request Form
• Remember to sign and date the form (incomplete forms will not be processed)
• $2.00 cash or money order per transcript
• Include a copy of a valid, government issued picture ID
• Mail the Transcript Request Form, copy of ID, and payment to:

• Transcript requests will be processed in 5 business days, after receipt of request, excluding delivery time
• A transcript will not be released to a third party without written consent from the student
• Transcripts are sent through the United Stated Postal Service
• Incomplete requests will not be processed
• We do not issue duplicate diplomas
