Transcript Request

If you would like a transcript, please download the Transcript Request Form and follow the instructions below.

You may also come into our office on the campus of Kauai High School and we can assist you in completing the request form. A cash payment of $2.00 per transcript is required.

Directions:

• Download the Transcript Request Form (below).
• Open with Adobe Acrobat Reader or print it out.
• Fill in the appropriate information and sign on the Requestor's Signature line.
• Attach a copy of a government issued picture ID (pdf or jpeg).
• Attach a money order (payable to DOE) of $2.00 per transcript.
