Transcript Request Instructions:

Fill out the Windward Campus Transcript Request Form below, print, and submit it with the required documents and payment.

Online Payment Option: Do not pay the transcript fee until you have submitted the Transcript Request Form and received verification from our office.

After receiving verification, click here to pay for your transcript online. Create an account or sign in, select Windward Campus, and then select Purchase Supplies to pay the transcript fee.
